Credit Card Authorization Form

I authorize Casablanca Event Rentals LLC to charge the credit card listed above for all agreed upon rental charges, including but not limited to rental fees, delivery fees, setup fees, additional rental hours, after hours pickup fees, damages, missing items, cleaning fees, late return fees, and any other applicable charges related to the rental agreement. I confirm that I am an authorized user of this card and that this authorization remains valid until all balances are paid in full.
